I authorize Curry's Collision Center to estimate and repair my vehicle unless it is deemed to be a total loss. I also or full bill if Insurance Company is not understand that I will be responsible to pay my deductible of $ reasonable. An express mechanic's lien is hereby acknowledged on the above vehicle should the need occur, to to pay Curry's Collision Center for secure the amount of balance remaining. I authorize supplemental repairs approved by Insurance Company. All supplement payments should be mailed to Curry's Collision Center, 1731 Hwy 16 West, Griffin, Ga. 30223 Tax ID# 581800446. I also do herby authorize Curry's Collision and its designated employees to sign my name on the Insurance drafts for the initial and supplementary payments that may arise before, during, and after repairs are completed to my vehicle.
All estimated repair days are subject to change. Curry's Collision Center is not responsible for delays outside of our control, including but not limited to parts delays, insurance approval, supplements, sublet repairs, etc. It is strictly understood Curry's Collision Center is not responsible for rental days, or upgrades accrued during the repair process of my vehicle. I authorize Curry's Collision to extract data that is stored in the vehicle including its event date rècorder and to conduct any scanning of diagnostics operation. Access to this data will be used for the purpose of conducting vehicle diagnostics and/or repairs.
There is a 4% charge for all debit or credit cards used.
